*Member+Child & Member+Family include the first child 5 years and up at a rate of $40. If 1st child is Newborn to 2, add an additional $50. If the 1st child is age 2 to 4, add $30. Additional children are added at the following rate: Under 2 yrs = $90; 2 – 4 years = $70; 5 – 21 yrs = $40

What is an Initial Unshareable Amount (IUA)?

It is the personal responsibility of members for their medical Needs. All qualified medical expenses after the IUA is met are 100% shareable with the Medical Cost Sharing community.
