QR Life Code and Personal Medical Records Vault

Store any pertinent information about your health and emergency contact information in our HIPAA-compliant, secure medical vault.

Our membership card has your QR Life Code number on it. If you become incapacitated, a first responder can scan the QR Life Code on your card, getting the important medical information they need to perform treatment immediately. You can allow our TeleHealth providers access to your Personal Electronic Medical Vault so they can store any records and charts for their reference. Zion Health Medical Cost Sharing

Contain costs for large medical needs, protecting you in case of an injury or illness. Read: Zion FAQ’s

  • Zion Health sends a welcome email containing a virtual membership card and, about two weeks later, mails you a letter and physical membership card.
  • There are two fees associated with Zion medical cost sharing: the monthly share fee and the initial unshareable amount or IUA you’ve chosen. Example:

As an example of how this process works, we’ll choose the $1000 IUA level. The member pays the first $1000 of a medical need and the community of members share 100% of any remaining costs with no cap. So, if a member is diagnosed with cancer, for example, the member will pay the first $1000 towards that need and then the community of member’s shares 100% of the remaining costs above that point. That will include all primary care and specialist visits, chemotherapy, radiation, physical therapy, x-rays, lab tests, medications and any other required treatment. The costs for every part of that treatment plan are included in that one need. And, unlike traditional insurance, the initial unshareable amount originally paid out is accepted for the entire duration of that need.

  • Determination of a “Need”, as defined by Zion Health, is the following: “Expenses related to the same medical condition, including those for separate incidents, will be shared as one need and accumulate towards the total need amount. If at least 12 months pass without any symptoms, medication or other treatment for the original condition creating that need, or related subsequent condition recurs, it will be treated as a new need.”
  • A member, couple or family is responsible to pay an IUA for up to three separate needs per year. For each additional need, the member is responsible for the first $500 in fees, after which all remaining bills are shared by the sharing community.

Alternative Medical Services
If a member has a medically diagnosed issue that can be treated less invasively, less costly and/or in less time, with alternative, naturopathic, or holistic solutions, the member can submit a treatment plan from a state-licensed alternative medical provider. If it meets the sharing guidelines and is approved by Zion Heath, the member will be able to seek out those services instead of being required to utilize the traditional healthcare system.

Pre-existing Conditions                           

Pre-existing conditions have a waiting or phase-in period:

  • 1st Year of Membership – Waiting period of all pre-existing conditions.
  • 2nd Year of Membership – Up to $25,000 of sharing for pre-existing conditions.
  • 3rd Year of Membership – Up to $50,000 of sharing for pre-existing conditions.
  • 4th Year of Membership and Beyond – Up to $125,000 of sharing for pre-existing conditions per year.

High blood pressure, high cholesterol and diabetes (type 1&2) will not be considered a pre-existing condition as long as you have not been hospitalized for the condition in the past 12 months and able to control through medication or diet.

International Travel
If you travel internationally and you get sick or injured, you do not need additional travel insurance. Instead, you may submit any medical bills you incur as long as they are written in English and are converted to U.S. dollars.

End of Life Assistance
For a Member, and/or his or her dependents, who die(s) after two years of uninterrupted Membership as a Sharing Member, financial assistance to the surviving family will be provided by the Members according to the following schedule, upon receipt of a copy of death certificate, and as listed on the Member enrollment.
$10,000 – Primary Member
$10,000 – Dependent Spouse
$2,500 – Dependent Child

Managing Medical Needs as a Self-pay Patient or Utilizingthe PHCS Network
As a medical cost sharing member, you are a Self-pay Patient. This allows you to utilize any Provider who accepts cash pay. When you contact a Provider’s office to schedule a consultation, you will tell them you are a self-pay patient and request the cash rate for the service you are seeking. Most providers typically offer a 30% to 50% discount from the retail rates. You are responsible to pay out-of-pocket up to your I.U.A. amount. If your bill is higher than your I.U.A., you will pay your I.U.A. amount and ask to be billed the remainder.
You will, then, submit that bill to Zion Health.

Zion Health also offers an alternative option. If you’d like to utilize a provider within the PHCS discount network, for preventive care, injury or illness, you will access the PCHS Provider Network Directory (our Concierge can help you with this task at your request), choose your provider and call the provider to be certain he/she is currentlyin the network.

When you arrive at the office, present your Zion Health Membership ID Card to your provider. The card contains all the billing information that your provider needs to apply the PHCS Networkdiscounts and send medical data to Zion Health for processing.

For Zion Health to control costs and limit over-charges we require providers to submit bills directly to Zion Health for processing. Zion Health DOES NOT reimburse members for payments made directly to providers for preventive services, injury or illness.

Zion will send your provider an EOS statement reflecting the discount price for your service. You will then get a bill from your provider, to whom you will remit payment.

Network members may contact Zion Health to make an exception if there are no in network providers conveniently located in their area.

Submission of Medical Needs
When you have a qualified medical need that you find will be in excess of your initial unshareable amount, you must submit a “NeedsRequest” form to Zion Health.
You will find this form at the following URL: https://zionhealth.org/needs-request/

All qualified medical expenses submitted within six (6) months of service and after the IUA is met, are shareable with the Zion Health community at 100% and without an annual or lifetime limit. Receiving the bills and needs request promptly will help Zion Health to provide the speedy processing of your need. When there is a number of bills related together in one need, it is helpful for these bills to be submitted at one time if obtained within a 30-day period. Original and itemized bills should be submitted to Zion Health with a need handling form as soon as possible.

Quality & Pricing Incentive
Zion Health offers an IUA discount incentive for scheduled pre-approved surgeries and procedures. If you are planning to schedule a surgery or other procedure that relates to a shareable need you may be eligible for a discount of up to 50% off your standard IUA. To qualify for this discount the following criteria must be met:

  • The surgery or procedure must be related to a shareable need.
  • The anticipated cost of the procedure must be more than your standard IUA amount.
  • The IUA discount must be pre-approved in writing by Zion Health.
  • By working with our MPB Concierge to schedule surgeries and procedures in advance through our Bluebook Care Connect program, surgical packages of $5000+ can be negotiated and recommended that provide both high quality service and fair pricing for the Zion Health Community.

Healthcare Bluebook

If you need a provider, diagnostic center, hospital or surgical procedure, you can call your Concierge directly to search for these providing services that offer the highest quality at the lowest cost.

Life Care (MAP / EAP)

Beginning January 1, 2021, you may utilize this service for behavioral health issues as well as lifestyle needs including excessive stress, anxiety, depression, family-related issues, substance abuse or any other problem area.

Up to four virtual, face-to-face or telephonic sessions with a licensed counselor are available with a two-month hiatus between each four session segment. Additional resources are available such as legal, financial, support groups, housing, elder care, etc.

Healthcare Discount Program

WholeHealth Living Choices is an online complementary and alternative medicine (CAM) discount network with over 35,000 providers in over 40 CAM specialties.

Members receive 10 – 50% off cash services and don’t need to get a referral, pre-approval or handle a claim. They offer health solutions for mind and body, diet and nutrition, movement and personal training, dental, hearing and vision, physical and occupational therapy.

This service begins January 1, 2021. To get started, visit the MPB.Health website. In the “Already A Member” tab, click the “WholeHealth Living Choices” link. Register for the site, download the offers of your choice and redeem them as instructed. For Complementary and Alternative Medicine Providers, refer to the Network Directory and call the Provider of your choice to find out discount rates for the services you need.

If you travel internationally and you get sick or injured, you do not need additional travel insurance. Instead, you may submit any medical bills you incur as long as they are written in English and are converted to U.S. dollars.

End of Life Assistance
For a Member, and/or his or her dependents, who die(s) after two years of uninterrupted Membership as a Sharing Member, financial assistance to the surviving family will be provided by the Members according to the following schedule, upon receipt of a copy of death certificate, and as listed on the Member enrollment.
$10,000 – Primary Member
$10,000 – Dependent Spouse
$2,500 – Dependent Child

Managing Medical Needs as a Self-pay Patient or Utilizingthe PHCS Network
As a medical cost sharing member, you are a Self-pay Patient. This allows you to utilize any Provider who accepts cash pay. When you contact a Provider’s office to schedule a consultation, you will tell them you are a self-pay patient and request the cash rate for the service you are seeking. Most providers typically offer a 30% to 50% discount from the retail rates. You are responsible to pay out-of-pocket up to your I.U.A. amount. If your bill is higher than your I.U.A., you will pay your I.U.A. amount and ask to be billed the remainder.
You will, then, submit that bill to Zion Health.

Zion Health also offers an alternative option. If you’d like to utilize a provider within the PHCS discount network, for preventive care, injury or illness, you will access the PCHS Provider Network Directory (our Concierge can help you with this task at your request), choose your provider and call the provider to be certain he/she is currentlyin the network.

When you arrive at the office, present your Zion Health Membership ID Card to your provider. The card contains all the billing information that your provider needs to apply the PHCS Networkdiscounts and send medical data to Zion Health for processing.

For Zion Health to control costs and limit over-charges we require providers to submit bills directly to Zion Health for processing. Zion Health DOES NOT reimburse members for payments made directly to providers for preventive services, injury or illness.

Zion will send your provider an EOS statement reflecting the discount price for your service. You will then get a bill from your provider, to whom you will remit payment.

Network members may contact Zion Health to make an exception if there are no in network providers conveniently located in their area.

Submission of Medical Needs
When you have a qualified medical need that you find will be in excess of your initial unshareable amount, you must submit a “NeedsRequest” form to Zion Health.

All qualified medical expenses submitted within six (6) months of service and after the IUA is met, are shareable with the Zion Health community at 100% and without an annual or lifetime limit. Receiving the bills and needs request promptly will help Zion Health to provide the speedy processing of your need. When there is a number of bills related together in one need, it is helpful for these bills to be submitted at one time if obtained within a 30-day period. Original and itemized bills should be submitted to Zion Health with a need handling form as soon as possible.

Quality & Pricing Incentive
Zion Health offers an IUA discount incentive for scheduled pre-approved surgeries and procedures. If you are planning to schedule a surgery or other procedure that relates to a shareable need you may be eligible for a discount of up to 50% off your standard IUA. To qualify for this discount the following criteria must be met:

  • The surgery or procedure must be related to a shareable need.
  • The anticipated cost of the procedure must be more than your standard IUA amount.
  • The IUA discount must be pre-approved in writing by Zion Health.
  • By working with our MPB Concierge to schedule surgeries and procedures in advance through our Bluebook Care Connect program, surgical packages of $5000+ can be negotiated and recommended that provide both high quality service and fair pricing for the Zion Health Community.

Healthcare Bluebook

Life Care (MAP / EAP)

Beginning January 1, 2021, you may utilize this service for behavioral health issues as well as lifestyle needs including excessive stress, anxiety, depression, family-related issues, substance abuse or any other problem area.

Up to four virtual, face-to-face or telephonic sessions with a licensed counselor are available with a two-month hiatus between each four session segment. Additional resources are available such as legal, financial, support groups, housing, elder care, etc.

WholeHealth Living Choices is an online complementary and alternative medicine (CAM) discount network with over 35,000 providers in over 40 CAM specialties.

Members receive 10 – 50% off cash services and don’t need to get a referral, pre-approval or handle a claim. They offer health solutions for mind and body, diet and nutrition, movement and personal training, dental, hearing and vision, physical and occupational therapy.

This service begins January 1, 2021. To get started, visit the MPB.Health website. In the “Already A Member” tab, click the “WholeHealth Living Choices” link. Register for the site, download the offers of your choice and redeem them as instructed. For Complementary and Alternative Medicine Providers, refer to the Network Directory and call the Provider of your choice to find out discount rates for the services you need.
